Very thankful for everyone there.

It was a very pleasant and informative visit. And I was given clear instructions concerning next years follow up exam and setting up a new referral appointment.

Dr. Kolich was fantastic. She listened to me and she was very clear and up front and I felt very heard and understood and yeah, it was just a very pleasant visit. What didn't go well is when I first checked in I was walking with a walker and visibly fatigued when I got to the station to check in and the lady kept asking me all these questions and I just asked if I could sit down and then she told everyone that I denied wanting to I denied all the check-in questions and wanting a pad and blamed me for that. So I just didn't appreciate that. She should have been more understanding of the situation I was in and offered to just bring me the iPad out to where I was sitting.

Scheduling appointments is hard and confusing

My provider is the best!! Everyone at the clinic is excellent!!

Dr Kolascz is outstanding!

Nice staff. They answer questions you have and and make sure you have mentioned everything you needed to during the phone appointment. The doctor listens to any concerns and provides good care.

I was kind of expecting a little rebuke from the receptionist because I wasn’t early. I was exactly on time but was apologetic for not being there earlier and her friendly response was “Oh no, you are great, right on time.” My overall experience was very positive.

Kira is the lovely person who checked me in. She is efficient, thorough, and a very sweet young woman. I liked her very much!

My doctor was great! She took time, was a great listener, gave me confidence as I wait results and possible further treatment.

I had to wait five months for this appointment. That was the earliest I could get in when I scheduled. That was frustrating.

I trust Dr. Kolacz and will continue with her as primary doctor.

When I logged into MyChart in May to schedule the preventive visit with my primary care doctor for Aug., the first date available wasn’t until Oct. Having to make a routine appointment five months out isn’t patient-friendly!

As always, I left the clinic confident that I had received the best care possible!

The facility itself is pretty rundown, but the staff is excellent

My last several appts have been the best I’ve had in sometime. The care has been excellent.

The wait time was extremely long before my appointment started. This makes me reconsider the clinic. I didn't see the provider until almost an hour after my scheduled appointment time.

Dr. Kollatz is a tremendous doctor. She always listens carefully and answers all questions and covers all bases. I really feel fortunate to have her as my primary care physician. She is an excellent doctor and is a great value I would expect to Fairview System. That's all I have to say. Thank you.

Dr. Pamela Kolacz is a wonderful healthcare provider because she listens to me, she explains every fact and every test, she tells me the options, she's warm heart and caring and reassuring and she always handles the calling back and the reporting in a gracious way. I really commend Dr. Pamela Kolacz. She's an outstanding doctor for many, many years and I thank you, Fairview, for having me put in a close touch with such a wonderful provider.

Appointments are always great with Dr. Kolacz. That is why I prefer to drive a longer distance to see her.

Allways very attentive to my concerns.

I did have to wait a long time to be seen by my Dr. this time. My apt. was for 3:40 pm and I sat in the exam room after the intake for 30-40 min.

It's frustrating that you ask us to be there 10 minutes early for our appointment then you check us in and I get into the room on time for my appointment and then I sit in the room for 35 minutes waiting for my doctor. I understand things run long, but if my doctor is running behind, why am I not given a courtesy phone call that says hey Your doctor is running behind today why don't you come in 15 minutes later for your appointment. Then nobody is sitting around for a half hour getting frustrated and feeling unseen. Something to think about.

I did not like answering those questions on that tablet before the appointment. I have answered them the same as the last time.
